The Corazon Locsin Montelibano Memorial Regional Hospital in Bacolod City issued a public advisory this week alerting residents to the unauthorized use of its official medical certificates, prescription pads, and physicians' credentials. Hospital management said it discovered that forged documents carrying the institution's name and seal were in circulation, raising concerns about patient safety and medical fraud. Dr. Joan E. Cerrada, the hospital's chief, confirmed the advisory and urged the public to verify the authenticity of any medical documents purporting to originate from the facility. The hospital did not specify how the fraudulent documents were being used or the extent of the problem. Authorities have not yet announced investigations into the matter or identified those responsible for creating or distributing the counterfeit materials. The hospital advised patients and the general public to report any suspicious medical certificates or prescriptions to its management immediately.
