Dementia Singapore and the Lien Foundation have unveiled a comprehensive support programme designed to help persons with advanced dementia transition from institutional care settings to their homes. The initiative, known as PDS+, represents a significant investment in end-of-life care and family support services across the region.
The $3.3 million programme combines early palliative care interventions with practical home-based support, recognising the growing need for patient-centred care options. As dementia prevalence rises across Southeast Asia, the partnership addresses a critical gap in care continuity for families managing late-stage illness at home.
PDS+ will operate over a 12-month support window, providing medical, psychological and logistical assistance to facilitate the transition away from centre-based care. The initiative encompasses pain management, symptom control, and family counselling to ensure patients receive appropriate care in their home environment.
The collaboration reflects growing recognition among healthcare and social support organisations that dementia care models must evolve to meet patient preferences and family needs. By combining clinical expertise with community-level support, the programme aims to improve quality of life for both patients and caregivers during the final stages of illness.
