India bans two-ingredient drug combinations for children under four years

India's drug regulator has widened restrictions on a class of pharmaceutical combinations used in cough and cold remedies, following expert health assessments. The government directive applies to all fixed-dose combinations (FDCs) containing Chlorpheniramine Maleate and Phenylephrine Hydrochloride, requiring mandatory warnings that these medicines should not be administered to children under four years of age.

The latest order builds on an earlier April 15, 2025 notification that had restricted certain formulations of these two-ingredient combinations. The expanded restrictions now cover the entire category of such products across the pharmaceutical industry, bringing consistency to safety requirements.

The decision emerged from recommendations made by an expert committee and India's Drugs Technical Advisory Board (DTAB), which reviewed the risk-benefit profile of these medications for young children. Health authorities determined that such combinations carry potential risks for the under-four age group, while safer therapeutic alternatives are available in the market.

Manufacturers have been instructed to prominently display warnings on product labels and packaging to ensure caregivers and healthcare providers are fully informed of age-related restrictions. The directive is intended to prevent inadvertent administration of these drugs to vulnerable young children and reduce potential adverse health outcomes in this sensitive age group.
